Requesting conduct history: two routes to distinguish
Direct route — verification pending. Josh recalls a faster way to obtain conduct-history information and will identify it. File review remains a documented fallback and may take months in his experience; this is not a verified statewide turnaround time. We have not yet confirmed the direct route’s office, form or authority.
Ask whether your DOC-173 and retained disciplinary records can be supplied directly without a full file-review appointment. Keep this inquiry separate from requesting to inspect your entire file.
Documented file-review fallback
- Write the Records Office / Records Supervisor using your facility’s Interview/Information Request or approved kiosk process. Facility handbooks differ: RYOCF lists DOC-643; CVCTF lists DOC-761. Ask which current form applies to you.
- Request both a file review and copies. State the incarceration dates, prior facilities and specific records needed: DOC-173, retained DOC-9 reports, decisions and final appeal/correction records.
- Ask about appointments, copying costs and any DOC-184 approval. Keep a dated copy. Follow the local chain of command for unanswered requests.
- If denied, ask for a written reason and the applicable review procedure. Seek timely assistance; a records request does not extend appeal or grievance deadlines.
Code provisions to check
DOC 303.80(1) and (6)(g) address delivery of a conduct report and decision in contested major proceedings. DOC 303.87(3) covers retention of the report and due-process documents, with evidence maintained separately. DOC 303.86 requires appeal corrections and expungement, and limits retention/use of dismissed or not-guilty reports. These rules do not establish a universal free-copy process for old records.
Official Wisconsin administrative code: DOC 303

CVCTF handbook: facility request procedure
Sample request — transfer to your facility’s form
To: Institution Records Office / Records Supervisor From: ____________________ DOC number: __________ Facility: __________ Date: __________ I request copies of my complete DOC-173 Inmate Conduct Record, retained conduct reports (DOC-9), disciplinary decisions/dispositions and final appeal or correction records for my incarceration from __________ to __________, including records from prior facilities where maintained. I am gathering records for sentence-adjustment preparation under Wis. Stat. 973.195. Please tell me whether these records can be supplied directly without scheduling a full file review, the local request form, copying charge and required disbursement form. If a file review is required, please explain that process. If another office holds a requested record, please tell me where to direct the request. Please identify any record that is unavailable, withheld, or removed/expunged and the reason, to the extent you may disclose that information. Please confirm that corrections ordered on appeal and expungements have been applied. Signature: ____________________

What Records handles
DOC policy 302.00.14 provides for Records to complete CR-261, attach the complete DOC-173 (blank if there is no conduct history), include other active judgments and mail the submission. Individual ticket copies and the additional materials above help our review; they are not all automatically filed. Final inclusion is reviewed with you.
Keep duplicates. Supporting materials supplied to Records may not be copied or returned.
